Streamline effect of "MorePrivacy" option (WHOIS, LIST)
- Update documentation in ngircd.conf(5)
- LIST: Don't hide channels for IRC Ops when "MorePrivacy" is in effect
- WHOIS: Don't hide IP addresses/hostnames when "MorePrivacy" is in effect

Closes #198
